Two ingredients, two different problems

The two ingredients target different issues, and it is worth being clear that they are separate conditions rather than two aspects of one thing.

  • [Sildenafil](/ingredient/sildenafil) is a phosphodiesterase type 5 (PDE5) inhibitor. It acts on the blood-flow mechanism involved in erection, by slowing an enzyme that would otherwise break down the signalling molecule responsible for relaxing blood vessels during arousal.
  • [Dapoxetine](/ingredient/dapoxetine) is a short-acting selective serotonin reuptake inhibitor (SSRI). SSRIs as a class act on serotonin signalling; a recognised effect of serotonergic medicines is a delay in ejaculation, and dapoxetine was developed specifically around that property with a much shorter duration in the body than SSRIs used for depression.

Both are Schedule 4 prescription-only medicines in Australia. A combination product containing both is prescription-only for both reasons, and the TGA prohibits advertising prescription-only medicines directly to consumers.

Why combination products need extra care

A single tablet containing two active ingredients is convenient, but convenience has trade-offs that a prescriber weighs.

  • You cannot separate them. If one ingredient causes a problem, the combination cannot be adjusted independently.
  • The interaction profiles stack. Every interaction relevant to sildenafil applies, and every interaction relevant to an SSRI applies as well.
  • Two ingredients mean two side-effect profiles. Effects can overlap — dizziness, for example, is recognised for both — which can make them more noticeable together.
  • Suitability requires both conditions to be present and assessed. Taking an SSRI when premature ejaculation has not been assessed adds risk without addressing a problem.

For these reasons combination products are not simply a shortcut, and whether one is appropriate at all is a clinical judgement.

The sildenafil side: contraindications

  • Nitrates are an absolute contraindication. Glyceryl trinitrate, isosorbide products and the inhalants known as poppers must never be combined with a PDE5 inhibitor; the result can be a severe, potentially fatal drop in blood pressure.
  • Riociguat is contraindicated.
  • Alpha-blockers require careful medical supervision.
  • Significant cardiovascular disease — recent heart attack or stroke, unstable angina, uncontrolled hypertension, severe heart failure — must be assessed, since sexual activity is physical exertion.
  • Severe liver or kidney impairment and inherited retinal conditions such as retinitis pigmentosa are also relevant.

The dapoxetine side: cautions specific to SSRIs

The SSRI component brings its own set of considerations, and they are not intuitive if you are thinking of the product purely as an ED treatment.

  • Serotonin syndrome risk when combined with other serotonergic agents — other SSRIs or SNRIs, tricyclic antidepressants, MAO inhibitors, St John's wort, tramadol, triptans used for migraine, and lithium among them. This can be serious.
  • MAO inhibitors require a washout period; this is a prescriber-managed matter.
  • Fainting and orthostatic effects. Dizziness and syncope are recognised with dapoxetine, particularly on standing, and alcohol compounds this.
  • Mental health history matters. SSRIs are not recommended for people with a history of mania, bipolar disorder or significant depression without careful specialist input, and any new or worsening mood changes or suicidal thoughts should be reported urgently.
  • Cardiac conditions such as significant valvular disease, heart failure or conduction abnormalities are relevant.
  • Nausea, headache, dry mouth, insomnia and diarrhoea are recognised effects of the class.

The combined interaction list

Because both ingredients are metabolised by liver enzyme systems that many other medicines also use, the interaction list for a combination product is longer than for either alone. Commonly cited examples include certain antifungals, macrolide antibiotics, HIV protease inhibitors and some antidepressants.

The practical implication: a full medicines list is not optional here, and it should include over-the-counter products, herbal supplements such as St John's wort, and anything obtained online.

Urgent warning signs

Seek immediate medical care for:

  • Chest pain during or after sexual activity — and tell the treating team a PDE5 inhibitor has been taken, because nitrates cannot be used.
  • An erection lasting more than four hours. Priapism is an emergency.
  • Sudden vision loss, or sudden hearing loss or ringing.
  • Fainting or collapse, particularly on standing.
  • Agitation, confusion, fever, rapid heartbeat, muscle twitching or rigidity — possible signs of serotonin syndrome.
  • New or worsening thoughts of self-harm.

Registration, counterfeits and storage

Combination products of this kind are frequently sold through unregulated overseas channels. The TGA has warned that such products may contain the wrong ingredients or amounts, which is a particular concern where an SSRI is involved and the buyer may not realise they are taking one. Check the Australian Register of Therapeutic Goods, look for batch numbers, expiry dates and manufacturer information, and disregard any seller offering to skip the prescription. Store in original packaging in a cool dry place, away from sunlight and out of reach of children.

What to do instead of guessing

Erectile difficulty and premature ejaculation are separate conditions and often have different causes, including anxiety, relationship factors, thyroid problems and prostate conditions. They deserve to be assessed rather than bundled.
