What the product contains

Second, longer presence cuts both ways. If a side effect occurs, it may also persist longer than it would with a shorter-acting ingredient. That is one of several reasons the choice between ingredients is clinical rather than a matter of preference.

The mechanism, plainly

During arousal the body releases a signalling molecule that relaxes smooth muscle in the blood vessels of the penis, allowing blood to flow in and stay. An enzyme called PDE5 breaks that molecule down. Tadalafil slows the enzyme, so the relaxation signal persists.

Because tadalafil also acts on smooth muscle elsewhere, it has been studied and is used internationally for other conditions, including symptoms of benign prostatic hyperplasia and pulmonary arterial hypertension. That is a useful illustration that PDE5 inhibitors are cardiovascular-acting medicines, not lifestyle products — which is exactly why they are prescription-controlled.

Contraindications: the non-negotiables

  • Nitrates are an absolute contraindication. Angina medicines such as glyceryl trinitrate and isosorbide, and the inhalants known as poppers, must never be combined with tadalafil. The result can be a severe and potentially fatal drop in blood pressure. Serious but uncommon events include priapism, sudden hearing loss and sudden vision loss.

Seek urgent medical care for chest pain during or after sexual activity, an erection lasting more than four hours, or any sudden change in vision or hearing. If emergency treatment is needed, state clearly that a PDE5 inhibitor has been taken and when — nitrate-based emergency treatments cannot be given.

Interactions worth declaring

Tadalafil is metabolised by liver enzymes shared with many other medicines. Certain antifungals, macrolide antibiotics, HIV protease inhibitors and some epilepsy medicines can raise or lower the amount in your bloodstream; grapefruit juice is commonly mentioned as well.
