If you have read anything about tadalafil, you will have encountered the phrases "daily" and "as-needed". They describe two different formulation approaches that exist for the same molecule. This article explains what that distinction is as a matter of product design and regulation.
It does not explain how to take anything. Tadalafil is a Schedule 4 prescription-only medicine in Australia, lawful supply requires a prescription from a registered Australian prescriber, and the Therapeutic Goods Administration prohibits advertising prescription-only medicines directly to consumers. Why two approaches exist at all
Most medicines in the PDE5 inhibitor class are formulated around a single use case. Tadalafil is unusual because its half-life is substantially longer than that of the other molecules in the class — it remains present in the body considerably longer.
That pharmacological property opened a second design possibility. Rather than a formulation intended for episodic use, manufacturers could develop formulations at lower labelled strengths intended for a continuous regimen. Both approaches were subsequently studied and registered in various jurisdictions as distinct products with distinct labelling.
The key point: these are two different registered products, not two ways of using one product.
What distinguishes them as products
- Labelled strength. Continuous-regimen formulations are formulated at lower labelled strengths than episodic-use ones. That is a formulation decision reflected on the pack.
- Approved indication wording. The product information for each differs, because they were assessed for different patterns of use.
- Clinical monitoring context. A continuous regimen implies an ongoing prescribing relationship and review, which is a different clinical arrangement from an episodic one.
- Interaction management. Continuous presence of a medicine in the body changes how a prescriber manages other medicines that interact with it — alpha-blockers being the obvious example.

Why this is a prescriber's decision, not a preference
It is tempting to read the two approaches as lifestyle options — one for planners, one for spontaneity. That framing skips everything that actually determines suitability:
- Cardiovascular assessment. Sexual activity places demand on the heart, and a doctor's judgement about whether that demand is safe for you is part of responsible prescribing, before any formulation question arises.
- Concurrent medicines. An alpha-blocker for prostate symptoms, a blood pressure medicine, an antifungal, an antiretroviral — each changes the picture, and continuous presence of a PDE5 inhibitor changes it further.
- Kidney and liver function. Both affect how the body handles the molecule, and both bear on formulation choice.
- Underlying cause. A vascular cause, a hormonal cause and a psychological cause do not point to the same management. A formulation choice made before the cause is understood is a guess.
- Review. A continuous regimen is a commitment to an ongoing clinical relationship, not a purchase.

The safety picture is identical for both
Formulation approach changes nothing about the contraindication set:
- Nitrates are an absolute contraindication. Glyceryl trinitrate sprays, patches and tablets, isosorbide products and the recreational inhalants known as poppers must never be combined with a PDE5 inhibitor. The combination can cause a sudden, severe and potentially life-threatening drop in blood pressure. This applies equally to continuous and episodic formulations — and with a long-acting molecule, the window in which the interaction is relevant is correspondingly longer.
- Riociguat is contraindicated.
- Alpha-blockers require careful medical management because of the additive blood-pressure effect.
- Significant cardiovascular disease — recent heart attack or stroke, unstable angina, uncontrolled blood pressure, certain arrhythmias — requires assessment.
- Inherited retinal conditions and significant liver or kidney impairment must be disclosed.
Urgent warning signs
Seek urgent medical care for:
- chest pain during or after sexual activity — tell the treating team a PDE5 inhibitor is involved, because nitrates cannot then be given
- an erection lasting more than four hours, a medical emergency where delay can cause permanent damage
- sudden loss or reduction of vision, or sudden hearing loss or ringing
- signs of an allergic reaction such as facial swelling or difficulty breathing

There is a specific practical reason this matters more for a long-acting molecule. If you are prescribed any PDE5 inhibitor and later need emergency care, the treating team needs to know — because it rules out nitrates. With a long-acting formulation the relevant window is longer. Carry a record of your current medicines, and tell any clinician who treats you.
What to take to a consultation
Be ready to describe when the difficulty started and whether it came on suddenly, list every medicine and supplement you take, mention alcohol and sleep honestly, and raise any heart, blood pressure, kidney, liver or eye conditions. Ask what the assessment shows about your cardiovascular health generally, because erectile difficulty is often the first visible sign of a vascular problem.
