What the numbers describe

A number following a brand name states the amount of active ingredient contained in each tablet, in milligrams, as declared by the manufacturer. It is a content specification printed on packaging.

It is not a quality tier, a performance rating, or a recommendation. Tadalafil is manufactured across a range of labelled strengths because prescribing differs between individuals — not because higher numbers represent a premium version of the same thing.

Why comparing numbers is the wrong exercise

Every medicine has two relationships with the amount present in the body: one with its intended action and one with its unwanted effects. Those relationships are not identical, and past a certain point additional amount contributes more to the second than to the first.

For PDE5 inhibitors, the effects that tend to track upward are the vascular and musculoskeletal ones recognised for the class — headache, indigestion, flushing, nasal congestion, dizziness, and the back or muscle pain particularly associated with tadalafil. Someone selecting a higher number on their own initiative is not obtaining a better outcome; they are altering their own risk without anyone assessing it.

What a prescriber is actually weighing

The strength decision is not a single judgement made in isolation. A prescriber considers:

  • Cardiovascular status, including blood pressure and any history of heart disease or stroke.
  • Kidney and liver function, because both affect how the body handles and clears the ingredient.
  • All other medicines, since several can substantially change how much tadalafil circulates.
  • Age and general health.
  • Previous experience and tolerance, including any side effects encountered before.
  • The underlying cause of the symptom, which may need addressing in its own right.

They then review. Adjustment after a period of use — in either direction — is normal practice rather than a sign that something went wrong. That review loop only exists if you go through a consultation.

What does not change with strength

The safety framework attaches to the ingredient, not the number.

  • Nitrates remain an absolute contraindication at every strength. Glyceryl trinitrate, isosorbide products and the inhalants known as poppers must never be combined with a PDE5 inhibitor, because the combination can cause a severe and potentially fatal drop in blood pressure.
  • Riociguat is contraindicated at every strength.
  • Alpha-blockers require careful medical management regardless of strength.
  • Significant cardiovascular disease — recent heart attack or stroke, unstable angina, uncontrolled hypertension, severe heart failure — requires assessment before any PDE5 inhibitor is considered.
  • The prescription requirement is unchanged. Tadalafil is Schedule 4 in Australia at every strength and under every brand name, and the TGA prohibits advertising prescription-only medicines directly to consumers.

Interactions change the real amount, not the label

This is the point most easily missed. The number on the box describes what goes in; what matters clinically is how much ends up circulating, and that depends on everything else in your system.

Certain antifungals, macrolide antibiotics and HIV protease inhibitors slow the liver enzymes that clear tadalafil, raising the amount in the bloodstream even though the tablet has not changed. Other medicines, including some used for epilepsy, do the reverse. Grapefruit juice is commonly cited too.

The practical consequence is that two people taking a product with the same number printed on it can end up with quite different amounts in their bodies. That is a strong argument for a full medicines review with a prescriber or pharmacist, including supplements and anything purchased online.

Because tadalafil lasts longer, so does everything else

Tadalafil has a substantially longer half-life than sildenafil. The implication that gets less attention is that side effects and interaction risks also persist longer. A headache or an episode of dizziness has a longer tail, and the nitrate contraindication remains relevant for longer after the medicine was taken.

If you ever need emergency treatment for chest pain, tell the treating team that a PDE5 inhibitor has been taken and when — it directly changes which treatments are safe to give.

Urgent warning signs at any strength

  • Chest pain during or after sexual activity.
  • An erection lasting more than four hours — priapism is a medical emergency that can cause permanent damage.
  • Sudden loss of vision, or sudden hearing loss or ringing in the ears.
  • Fainting, severe dizziness, or signs of an allergic reaction such as facial swelling or difficulty breathing.

Counterfeit risk rises with the number

Higher-numbered products are a common target for counterfeiters, because the number is the marketing hook. The TGA has warned repeatedly that products in this category sourced from unregulated overseas sellers have been found to contain undeclared ingredients, incorrect amounts, or contaminants — meaning the printed number can be meaningless.

Sensible checks: whether the product appears on the Australian Register of Therapeutic Goods, the presence of a legible batch number and expiry date, manufacturer name and address, intact packaging and consistent printing. A seller willing to supply a prescription-only medicine without a prescription has already told you what you need to know.

Store tablets in original packaging in a cool, dry place away from sunlight, below the temperature stated on the pack, and out of reach of children.
