Vidalista and Cenforce are frequently compared as though they were competing versions of the same thing. They are not. They are brand names attached to two different active ingredients — tadalafil and sildenafil respectively — that happen to belong to the same drug class.

Both molecules are Schedule 4 prescription-only medicines in Australia. Lawful supply requires a prescription from a registered Australian prescriber, and the Therapeutic Goods Administration prohibits advertising prescription-only medicines directly to consumers. This article is educational background, and it contains no dosing, timing or usage guidance, because those decisions belong in a consultation.

The brands and the molecules

  • Cenforce products contain sildenafil citrate and are manufactured by Centurion Laboratories.
  • Vidalista products contain tadalafil and are manufactured by Centurion Laboratories.

What they share

Both are phosphodiesterase type 5 inhibitors. The class slows an enzyme that breaks down a signalling molecule the body produces during arousal, which allows the smooth muscle in blood vessels to remain relaxed for longer.

Three consequences follow for both molecules:

  • Neither creates desire. Arousal is still required for the pathway to be engaged at all.
  • Both carry the same headline contraindications, described below.
  • Both are prescription-controlled in Australia precisely because those contraindications are serious.

Where the two molecules genuinely differ

The differences are documented in the published product information for each ingredient, and they are pharmacological rather than promotional:

  • Half-life. Tadalafil has a substantially longer half-life than sildenafil, meaning it remains present in the body for considerably longer. This is the single largest difference between them.
  • Food interaction. Sildenafil's absorption is more affected by a heavy or fatty meal than tadalafil's.
  • Side effect emphasis. Both share headache, facial flushing, nasal congestion and indigestion. Back and muscle ache appear more prominently in discussions of tadalafil; visual disturbance appears more prominently in discussions of sildenafil. Neither list is exhaustive.

What the product ranges look like

Within each brand, the number on the pack is the labelled strength of the active ingredient — nothing more. The Vidalista range includes Vidalista 20 and Vidalista 40; the Cenforce range includes Cenforce 100. Which strength and which molecule are appropriate for a given person is a prescriber's decision made on the basis of medical history, current medicines and organ function — not something to deduce from a comparison table.

Why individual response varies

Two people with similar test results can respond quite differently to the same molecule. The underlying cause of the difficulty, the condition of the blood vessels involved, diabetes status, hormone levels, kidney and liver function, other medicines, alcohol, sleep and anxiety all contribute. It is entirely normal for a prescriber to reassess after a period rather than treat the first choice as final.

That variability is also why "which is better" has no general answer. Better for whom, judged on what, is a clinical question.

The safety set that applies to both

Because both are PDE5 inhibitors, the critical warnings are shared:

  • Nitrates are an absolute contraindication. Glyceryl trinitrate sprays, patches and tablets, isosorbide products and the recreational inhalants known as poppers must never be combined with either molecule. The resulting fall in blood pressure can be sudden, severe and life-threatening.
  • Riociguat, used for pulmonary hypertension, is contraindicated.
  • Alpha-blockers, commonly prescribed for prostate symptoms or blood pressure, require careful medical management.
  • Significant cardiovascular disease matters independently. Recent heart attack, recent stroke, unstable angina, uncontrolled blood pressure and certain arrhythmias all change the assessment, because sexual activity itself places demand on the heart.
  • Inherited retinal conditions such as retinitis pigmentosa, and significant liver or kidney impairment, must be disclosed.

Some antifungals, some antibiotics and HIV protease inhibitors can alter blood levels of both molecules. Give your prescriber and pharmacist a complete medicines list.

The urgent signs

Seek urgent medical care for:

  • chest pain during or after sexual activity — tell the treating team a PDE5 inhibitor is involved, because nitrates cannot then be given
  • an erection lasting more than four hours, which is a medical emergency
  • sudden loss or reduction of vision, or sudden hearing loss or ringing
  • signs of an allergic reaction such as facial swelling or difficulty breathing
